A recent study conducted by the University of Eastern Finland has identified three distinct body mass index (BMI) trajectories among individuals diagnosed with type 2 diabetes. According to News Medical, the research evaluated electronic health records over a ten-year follow-up period to examine how these various weight patterns relate to comorbidities, diabetes medications, and healthcare service utilization.
Study Identifies Three Distinct BMI Trajectories in Type 2 Diabetes
The study included 3,539 adults living in North Karelia, Finland, who received a type 2 diabetes diagnosis between 2011 and 2014. Researchers categorized the participants into three specific groups based on their long-term BMI progression:
- Stable BMI trajectory: Accounted for 77.7% of the participants.
- Increasing BMI trajectory: Accounted for 14.6% of the participants, showing a steady rise in BMI throughout the follow-up.
- Decreasing BMI trajectory: Accounted for 7.7% of the participants, starting with a high BMI at baseline that decreased over time to levels close to the stable group.
Comorbidities and Medication Use Linked to Increasing BMI
The findings demonstrate that people with type 2 diabetes do not form a homogeneous group, as different trajectories carry varying clinical implications. Participants in the increasing BMI trajectory experienced a clustering of comorbidities during the follow-up period. Specifically, they suffered from more chronic kidney and urinary tract diseases, sleep disorders, sleep apnea, and chronic respiratory diseases compared to individuals in the other trajectory groups.
Furthermore, the utilization of diabetes medications that support weight management became increasingly common throughout the follow-up, most notably among the cohort with an increasing BMI trajectory. News Medical notes that first author Suvi Koivunen, a lecturer at the University of Eastern Finland, explained that identifying these trajectories helps isolate individuals who face an elevated risk of developing additional health complications and requiring more intensive care.
Complex Implications of Weight Loss in Diabetes Care
The study also revealed distinct outcomes for patients following a decreasing BMI trajectory. Individuals in this group exhibited higher healthcare service utilization and higher mortality rates than those in the stable and increasing groups. Additionally, they demonstrated a higher prevalence of memory disorders, as well as cardiovascular and pulmonary diseases.

Researchers emphasized that weight loss does not always indicate better health. While weight reduction can reflect successful management, it can also be tied to underlying illnesses, aging, and frailty. Because the register-based data could not determine the precise reasons for weight loss with sufficient confidence, the authors recommend careful evaluation of weight changes in clinical settings.
