GLP-1 medications may increase risk of hair loss in some men, study finds

Adults taking popular GLP-1 weight-loss and diabetes medications face an increased risk of hair loss, according to separate studies. Researchers point to a 7 percent added risk for men with a genetic predisposition, alongside metabolic changes driven by caloric restriction and rapid weight reduction.

Men who carry a genetic predisposition for the most common form of hair loss face a measurable increase in shedding when prescribed medications like Ozempic, Wegovy, and Zepbound, a new study published online in the Journal of Investigative Dermatology shows. Led by researchers at NYU Langone Health, the investigation uncovered the first genetic link connecting glucagon-like peptide-1 receptor agonist activity directly to male-pattern hair loss.

While many patients have reported thinning hair since the Food and Drug Administration first approved Ozempic in 2020, researchers had previously attributed the shedding to rapid weight loss. The new findings suggest a biological mechanism operates alongside metabolic changes, establishing a scientific link between the drugs and androgenetic alopecia.

Genetic Susceptibility and the Seven Percent Added Risk

The NYU Langone team focused their investigation on the GLP1R gene, which naturally elevates levels of GLP-1 receptor proteins in the human body. By comparing the presence of these genes with genetic markers found in men experiencing androgenetic alopecia—an inherited form of hair loss following a pattern along the top and front of the scalp—researchers calculated a 7 percent added risk of hair loss for genetically susceptible men using the medications.

To confirm the validity of this association, the study authors adjusted their models to account for hypertension, a condition thought to reduce blood flow to the scalp and harm hair follicle growth. They also checked their risk analysis against insulin resistance and lowered testosterone levels. In every adjustment, the increased risk held steady at 7 percent.

The research team utilized data from large public genetic repositories, including the eQTLGen database comprising 31,684 mostly White men and women, and the Complex Traits Genetics group database containing 205,327 mostly White men. Androgenetic alopecia is estimated to affect half of White American males over age 50, and almost the same number of White American females over age 50, predominantly those who are postmenopause. Surveys indicate that approximately 12 percent of Americans have used GLP-1 drugs strictly for weight loss, a figure that includes one-fifth of women aged 50 to 64.

Parallel research published in the journal BMJ provides a broader clinical look at the issue. Investigators at the University of Pennsylvania Perelman School of Medicine examined electronic health record data spanning from January 2019 through September 2024, comparing patients newly prescribed GLP-1 drugs against those taking other types of drugs that lower blood sugar.

Over an average follow-up period of 2.7 years, GLP-1 usage was associated with a higher overall risk of alopecia. The Penn Medicine team suggests that much of this shedding stems from metabolic changes rather than a direct side effect of the drugs.

When users restrict caloric intake, the resulting physiological stress and nutritional deficiencies in iron, zinc and biotin can lead to non-scarring alopecia—a type of hair loss where follicles remain intact and regrowth is often achievable. These deficiencies commonly accompany rapid weight reduction and can disrupt hair growth. Because these cases are typically reversible, clinicians can better reassure patients who experience sudden thinning after starting treatment.

The convergence of genetic findings and electronic health records points toward new preventative strategies for patients beginning weight-loss therapy. Dr. Petukhova noted that future experiments could allow clinicians to screen and benchmark patients for their risk of hair loss before prescribing GLP-1 medications.

Such screening protocols could lead to combination treatments, pairing weight-loss drugs with treatments to prevent hair loss such as minoxidil for patients identified as high risk. Researchers stress that further research is needed to understand the biological mechanisms through which GLP-1 use weakens hair follicle growth, alongside plans to investigate whether the links seen in men apply to women.
