The “light BioAge” index identifies risks that chronological age alone often misses.
Chronological age is the primary risk factor for cognitive decline, but it fails to account for the varying rates at which individuals age biologically. A new study utilizing data from the China Health and Retirement Longitudinal Study (CHARLS) suggests that a simplified biological aging index, termed light BioAge
, can more accurately flag the risk of future cognitive problems in adults aged 45 and older.
The four metrics of light BioAge
Unlike complex biological aging models, such as DNA methylation clocks or organ-specific measures that require intricate biomarkers, light BioAge relies on four routinely available metrics.
- Chronological age
- Creatinine
- Blood glucose
- C-reactive protein (CRP)
This index is designed for straightforward calculation and has already demonstrated strong associations with frailty and mortality. The recent research focused on whether this same simplified score could predict the onset of cognitive impairment—a decline in memory, reasoning, attention, and language that exceeds typical age-related changes.
Data from the CHARLS cohort
Researchers tested the association between light BioAge and cognitive outcomes using a nationally representative cohort of Chinese adults. The study drew from the China Health and Retirement Longitudinal Study (CHARLS), which recruited approximately 17,000 participants across 450 communities and 150 counties.
The study’s design followed participants from a baseline period in 2011–2012 through subsequent follow-ups in 2013, 2015, 2018, and 2020. To isolate the predictive power of the biological age score, the researchers only included participants who had no baseline cognitive impairment or memory-related diseases at the start of the study.
Cognitive impairment prevalence and risk factors
The study highlights the significant burden of cognitive decline, noting that mild cognitive impairment affects roughly 15.6% of community-dwelling adults aged 50 and older globally. In China, this prevalence is estimated to be higher, at 19.6%.

These factors contribute to brain changes such as hippocampal atrophy, increased inflammation, and an imbalance in amyloid beta, which increase neuronal vulnerability in areas of the brain critical for memory.
The finding that light BioAge can flag these risks suggests that clinicians may be able to identify high-risk individuals more effectively by looking at the body’s biological state rather than the calendar date of birth.
