Men with nonmetastatic prostate cancer who consume diets high in saturated and animal fats face a substantially higher risk of long-term mortality, primarily driven by cardiovascular disease, according to a Harvard T.H. Chan School of Public Health study published in JAMA Network Open.
But long-term survivorship brings distinct vulnerabilities, particularly to the heart and circulatory system. A major prospective investigation published in JAMA Network Open reveals that post-diagnosis dietary fat intake exerts a powerful influence on overall life expectancy. The findings show that small, intentional shifts in what is on the plate can meaningfully alter health outcomes long after treatment ends.
The Health Professionals Follow-Up Study Tracks Nearly 5,000 Men Over Decades
The study utilized data from the Health Professionals Follow-Up Study, a prospective cohort of male health professionals across the United States that has tracked participants since 1986. Participants were followed through December 2022, yielding a median follow-up period of 12.8 years.
The cohort had a mean age of 69.5 years at diagnosis, with 97 percent of participants self-reporting as White. To evaluate dietary impact while avoiding reverse causation from immediate post-diagnosis lifestyle changes, researchers defined post-diagnosis fat intake using food-frequency questionnaires collected at least six months after diagnosis, and updated those details every four years. During the multi-decade tracking period, 3,040 deaths from any cause were confirmed. The single largest category of fatalities—803 cases—stemmed from heart disease.
Animal and Saturated Fats Linked to Elevated All-Cause Mortality
The analysis established a clear statistical gradient between heavy saturated and animal fat consumption and shorter post-diagnosis lifespans. Men in the highest quintile of saturated fat intake faced a 24 percent greater risk of all-cause death compared with those in the lowest quintile, alongside a linear trend. In absolute terms, the estimated 10-year risk of mortality was 23.4 percent for patients in the lowest quintile of saturated fat intake, rising to 26.4 percent for those in the highest quintile—a absolute risk difference of 3 percentage points.


Crucially, the elevated mortality risk was driven primarily by non-cancer causes and cardiovascular complications rather than prostate cancer progression itself. Patients in the highest quintile of saturated fat consumption experienced a 42 percent greater cardiovascular mortality risk compared to those in the lowest bracket. Animal fat, serving as a primary dietary source of saturated fats, was similarly linked to higher all-cause mortality and deaths from other cancers.

Simple Dietary Substitutions Offer Measurable Survival Benefits
Rather than advocating for extreme or restrictive diets, the research team focused on substitution models—evaluating what happens when specific calories from animal sources are swapped for plant-based alternatives.
Similarly, replacing five percent of daily calories from saturated fat with monounsaturated fats—such as those found in olive oil and avocados—corresponded with a 20 percent reduction in all-cause mortality risk. Patients in the highest quintile of monounsaturated fat intake maintained a substantially lower cardiovascular mortality risk than those in the lowest intake tier.
“The central message is not simply to ‘eat less fat.’ Rather, the type and source of fat appear to matter. Replacing some animal fats and saturated fats with plant-based and unsaturated fats may help support long-term health after a prostate cancer diagnosis.”

Broadening Survivorship Care Beyond Initial Tumor Control
Protecting heart health through diets richer in plant-based, monounsaturated, and polyunsaturated fats forms an essential pillar of comprehensive survivorship management.

While the study demonstrates robust statistical associations, the authors caution that observational data cannot prove direct causation. Furthermore, researchers noted that participants consuming higher levels of saturated and animal fats were also more likely to smoke, maintain a higher body mass index, exercise less frequently, or manage type 2 diabetes. Even so, public health experts highlight that prioritizing accessible, everyday adjustments remains a practical tool for patients navigating life after cancer treatment.
